About our partnership
Covering Broxtowe, Gedling, Hucknall and Rushcliffe, South Nottinghamshire Place-Based Partnership is a collaboration of health and care providers, local authorities, voluntary groups and organisations and local communities working together to help people live happier, healthier lives. The partnership is built around six Primary Care Networks, or PCNs, which are groups of community services working alongside GP practices. You can find out more about the partnership here.

Gedling health and wellbeing event
Two of our Gedling Primary Care Networks are holding a health and wellbeing event on Saturday 14 October at Westdale Lane Community Centre in Mapperley!

The event, which runs from 11am to 2pm, offers some free healthy foodie treats, an opportunity for a quick health check, cost of living advice, fun activities for the kids and lots more. Find out more.
Calverton health and well-being event
Hot on the tails of the Mapperley health and wellbeing event, is another Gedling event happening at Calverton Village Hall on Saturday 21 October (10.00am – 3.00pm) 

Organised by the Calverton Patient Participation Group, this is a free event and will host over 20 providers of health and well-being advice and support. You will also find: 
  • Information, advice and free health checks
  • Informative talks throughout the day - 'Using the Community Defibrillator', 'Pillars of Nutrition', 'The Memory Café' and 'An Introduction to Mental Health & Wellbeing'.
  • Covid vaccination drop-in clinic – over 65s

Integrated Care Board visit to Cotgrave
On Monday 2 October, Nottingham and Nottinghamshire Integrated Care Board’s Chair Kathy Mclean and Chief Officer Amanda Sullivan joined Place Based Partnership colleagues Dave Banks, Deputy Chief Executive and Derek Hayden, Communities Manager at Rushcliffe Borough Council for a tour of Cotgrave to look at some of the fantastic improvements and community work happening in the town

Cotgrave is one of the areas we are working with as part of the integrated neighbourhoods programme. Successful integrated neighbourhood working happens when local health and social care, community groups and voluntary sector organisations work together with communities to find ways to improve the health and wellbeing of local people and there’s lots of great things to build on in Cotgrave. 

Starting at the state-of-the-art Cotgrave Hub, representatives from Rushcliffe Primary Care Network, Rushcliffe Community and Voluntary Service (CVS), Nottinghamshire Healthcare and the South Notts Place-Based Partnership team joined the tour. Dave gave an overview of the new hub and talked about some of the key regeneration work that had happened in the town.

Winter vaccinations - eligibility and how to book
There are some things you can do to help yourself stay well in winter. This includes getting your flu and COVID-19 vaccinations.

Flu and COVID-19 spread more easily in winter and can cause you serious illness.

Vaccination is the best way to protect yourself, those around you and the NHS in the colder months.

Community, voluntary and primary care
Killisick Community Fun Day
On Saturday 30 September, the South Notts Place-Based Partnership, Gedling Borough Council, Positively Empowered Kids and other local partners hosted a fun day in Killisick for local families. 

It was a fabulous day with something for everyone, adults and children alike, with a smoothie bike, hand massages, cake stalls, jewellery, Inflatable slide, skateboarding, face painting, arts and crafts, blood pressure checks, local GPs (pictured above) providing health advice and much more!

Killisick, in Geding, is one of the five pilot areas in Nottinghamshire and Nottingham to be funded through the NHSEI Prevention Programme to promote happy and healthy communities in areas where the greatest health inequalities exist.

New memory cafe in Stapleford!
As featured in this newsletter earlier this year, as part of the Community Care Transformation Programme the Nottingham West team were approached by local residents of Stapleford, who wanted a memory café in the town. 

As it had also been identified as a gap in service provision during the Dementia Holistic Pathway work, the Local Design Team were keen to get started. 

On Wednesday 13 September, the first memory cafe took place with an informal meeting in Wetherspoons pub, with lots of coffee, chat and support. The meeting was buzzing with stories and ideas for future meetings and venues. A great time was had by all and Jayne, a local resident who will be leading the work, crocheted everyone a forget-me-not as a welcome.

This is a co-produced piece of work led by local residents and councillors. Currently residents are setting up a committee and, with support from the Local Design Team and partners, will get a bank account and charity status.

Local team crowned winners at National Awards!
Well done to Nottingham West pharmacy-led cardiology team, who are were crowned winners in the 'Integrated and joint working' category for their project 'Improving hypertension case finding, diagnosis and treatment' at the October 2023 PresQIPP awards.
Shortlist announced for first ever ICS Health and Care Awards
After receiving a150 entries from across the health, care and voluntary sector, the shortlist for the first ever ICS Health and Care Awards has been announced.

The awards have been organised to celebrate success across the Integrated Care System – a partnership of NHS services, local authorities, public sector and voluntary organisations.

Gedling Falls Prevention Project shortlisted in ICS awards
The Gedling Falls Prevention Project has been shortlisted in the Prevention Award at this year's ICS Health and Care Awards.

Evidence highlighted that physical and mental health deteriorated during the pandemic, and there had been an increase in falls amongst older adults. 

Partners across Gedling, including Gedling Borough Council, Arrow and Synergy Primary Care Networks, Active Notts and a local health coach came together to harness the power of their local knowledge connections and networks to identify areas of need and discuss possible solutions. 

They identified that falls prevention and management classes led by a postural stability instructor have been proven to help people at risk of falling to increase and
maintain their strength and balance, improve their understanding of how to get up from the floor and increase people’s social activity and mental wellbeing.

An incredible 70% of people attending reported an increase in their strength and balance and 97% increased knowledge of how to get up from the floor on their own. They have been empowered to lead a more independent life and they have become more socially active.
Community heart failure project shortlisted in ICS health and care awards

Our second team shortlisted in the ICS Awards is the Community Heart Failure Project, which is up for an award in the Best Outcomes category. 

In September 2021, a Heart Failure Transformation Group (HFTG) was established to consider how to address the backlog of heart failure patients due a review across the Gedling and Hucknall PCNs. It was also tasked with re-modeling the heart failure pathway based on learning, integrated working and NICE recommendations

After securing funding from NHS England in 2022, a clinical pharmacist with a special interest in heart failure was seconded to support the Primary Care Networks to reduce the backlog.

The clinical pharmacist, working alongside a heart failure specialist nurse and consultant cardiologist, offered clinics to increase medication levels where needed, review heart failure patients and identify clinical risks and support the heart failure specialist nurse in her role.

As a result of the clinical pharmacist being in post for a year, the backlog of patients was reduced from 100 patients to around 17 patients and the waiting time to see a heart failure specialist nurse was down from 52 weeks to approximately seven weeks.
Trent Bridge Community Trust Winter Wellness sessions
Winter Wellness is for people living with dementia and their carers, as well as people with mental health problems who want to get out over the winter months. Sessions are delivered in a peaceful setting and provide weekly activities to promote improvements in physical and mental wellbeing.

Winter Wellness takes place every Tuesday from 1.30-3pm at Cotgrave Community Garden (opposite Cotgrave Leisure Centre, NG12 3PJ). Depending on the weather, they also use Cotgrave Leisure Centre’s inside space for wellbeing activities.

Support for your mental health
World Mental Health Day took place on Tuesday 10 October and was an opportunity to raise awareness and make a positive change for everyone’s mental health. The theme for 2023 was ‘Mental health is a universal human right’

Around 1 in 4 people experience mental health problems every year. It is a widespread and common issue affecting people from all walks of life, and yet we often find it hard to talk about.

Across South Nottinghamshire, there are a number of ways you can get support, whether that's support from a social prescriber or mental health practitioner via your GP Practice, or more specialist support if you're facing a mental health crisis. You can find details of some of the support below...
