Young People’s Wellbeing Practitioner and Social Prescriber

Grade: Band 5 - £29,970 - £36,483 per annum (pro rata for part time)

Due to the development of a new service, we’re excited to offer an opportunity for a Wellbeing Practitioner or Social Prescriber to join a new multi-agency team supporting young people aged 16 and over in a supported accommodation setting. This innovative approach builds on Nottinghamshire County Council’s current 16+ Supported Accommodation model and introduces a dedicated multi-disciplinary team (MDT) delivering specialist, psychologically informed care. The service has been shaped in partnership with Public Health and Nottinghamshire Healthcare NHS Foundation Trust, and is part of the successful DfE-funded Staying Close Offer – designed to give young people leaving care the best possible chance to thrive.

Research highlights that many young people in supported accommodation have experienced significant childhood trauma, which can impact their ability to regulate emotions and communicate their needs. Around 75% are also affected by mental health or substance use challenges. These experiences can lead to behaviours that are difficult to manage and, in some cases, risk placement breakdowns. While supported accommodation providers offer essential housing and care, they are not specialist services. The addition of wellbeing and mental health professionals to the team will ensure young people receive tailored, therapeutic support – helping them maintain stability and build a positive future.

Hosted by Nottinghamshire Healthcare NHS Foundation Trust, this home-based role is an exciting opportunity to join the national Skills Development Network (SDN) team. The post plays a key part in delivering the East Midlands SDN Programme and the Midlands Digital Programme, working closely with the East Midlands SDN Steering Group and the Midlands Digital Board. While primarily remote, there will be occasional travel within the Midlands to support regional activity.

This varied administrative role involves supporting a wide range of development offers across finance, procurement, and digital health. You’ll work alongside the wider SDN team to ensure programmes and events run smoothly and are delivered to the highest standard. From organising events to supporting project delivery, your contribution will help ensure the successful implementation of initiatives that develop skills and improve services across the region.
