About our partnershipCovering Broxtowe, Gedling, Hucknall and Rushcliffe, South Nottinghamshire Place-Based Partnership is a collaboration of health and care providers, local authorities, voluntary groups and organisations and local communities working together to help people live happier, healthier lives. The partnership is built around six Primary Care Networks, or PCNs, which are groups of community services working alongside GP practices. You can find out more about the partnership here.

Local Communities Fund now open for grant applications
|
|
|
|
Groups and charities across Nottinghamshire can now apply for the latest round of community-improving grants. These funds help towards boosting local facilities, essential running costs, as well as improving residents’ access to affordable and healthy food. It’s all part of Nottinghamshire County Council’s continued commitment to encourage healthy and sustainable communities by financially supporting community groups and charities. The latest round of the Local Communities Fund (LCF) is open for applicants until Friday 24 May 2024. Eligible groups can apply for funding which demonstrate that their projects will help communities live healthier and more independent lives. For more details, please see the County Council website.

New bereavement support groups are a 'life line' for Mel
In Rushcliffe, where there was a significant need for bereavement support, people were finding that traditional avenues were difficult to access and mainstream services lack the specialised bereavement support.
In light of these challenges, the Rushcliffe Social Prescribers set up bereavement support groups, providing an invaluable resource for the community. By drawing upon their own lived experiences, the team has developed peer support groups that provide solace, understanding, and companionship to those navigating the complexities of grief.
Through these peer support groups, people find strength in their vulnerability and solidarity in their shared experiences.
South Notts News talked to Mel Watson, 76 from East Leake (pictured right) who has been attending the groups now for over 18 months, after sadly losing his wife two years ago.

Mel said: "The group has been a life saver for me, it gave me confidence and something to look forward to. The support has been absolutely wonderful and I have made genuine friends. I don’t know what I would have done without it. My family are really pleased that I enjoy going and get so much from it.” Read Mel's story and find out more about the groups here.

Award-winning work of local health team recognised on the national stageThe Nottingham West Primary Care Network (PCN) Pharmacy Cardiology team shared their success story on the national stage at the recent NHS Confederation Primary Care Conference. The conference, held in London, highlighted opportunities to really show what primary care can do if it is supported and empowered. Beth Rushton, Senior Clinical Pharmacist and Clare Watson, Health Inequalities Lead, were asked to speak about the PCN’s award winning hypertension service as part of a session entitled Unlocking the potential: tackling local health inequalities through greater collaboration between general practice and community pharmacy. The session included a panel discussion chaired by Professor Bola Owolabi, Director of the National Healthcare Inequalities Improvement Programme at NHS England. How could advice on prescription help you?Advice on prescription aims to improve mental health and wellbeing by supporting people with non-medical issues that may impact their health. The team at Citizen’s Advice in Broxtowe can provide practical information, advice and ongoing support to patients who are experiencing problems such as housing, employment, welfare benefits, rising debts, money management and other practical matters. A carer recently needed support, and turned to his GP for help. His GP put him in touch with a social prescriber, which led him to the team at Citizen’s Advice. He is a full-time carer for his wife who suffers from severe Crohn's disease and has had much of her bowel removed. He was exhausted and sleep-deprived and now suffers stress and anxiety as well as some physical health problems. A social prescriber referred him to Citizens’ Advice where he got support to access the benefits he was entitled to and practical advice such as how to apply for a blue badge. Revolutionising menopause support in Rushcliffe In Rushcliffe, they’re revolutionising the approach to menopause healthcare! The innovative initiative, involving GP-led support group menopause sessions, with social prescribing support, has just concluded its pilot run with extraordinary success.
The groups were set up as a way to allow women more time to discuss these issues with GPs, and allow for the needs of people in the same situation to be met together.
The response has been overwhelmingly positive. Women participating in the programme report smoother access to GP advice, a profound sense of being heard, and a strong support network, alleviating the isolation often felt during menopause. Clinicians and patients alike applaud the efficiency and effectiveness of this approach.

Arrow and Synergy hold joint community engagement event
|
|
|
Arrow Health and Synergy Health Primary Care Networks (PCNs) held their second face to face 'Joint Community Engagement Group' meeting on Wednesday, 10 April at Westdale Lane Community Centre, in Mapperley.
A wide range of community groups and providers now attend the meetings, including Daybrook Bowls Club, Positively Empowered Kids, Your Health Notts, Citizens Advice, Social Services – Gedling Aging Well Team, Wayfinder Advocate, The Ark Gedling Welfare Assistant, Patient Participation Representatives, PCN Social Prescribing Leads, PCN Health Inequality Leads and many more!
The group are currently working on their 2024 / 25 priorities, which are ‘Partnership Working’ and ‘Social Isolation’ They discussed how to identify positive interventions, develop services and close gaps in services in a co-productive way, with the health and wellbeing of local people central to this.

West Bridgford dementia information event a ‘huge success'Over 100 people attended a dementia information event organised by Carefound Home Care, with over £300 raised for Rushcliffe Dementia Action Network (RDAN). The team from Carefound Home Care in West Bridgford organised a dementia information event at Gamston Community Hall earlier this month in partnership with Rushcliffe Dementia Action Network (RDAN). The event presented a relaxed opportunity for people to learn about local groups and organisations available to help people living with dementia and their carers. Over 100 people attended on the day and there were stalls from a range of groups including Alzheimer’s Society, Bloomin Dementia, FarmEco, Flintham Museum, Forget Me Notts and Memory Lane Café. Information sessions were also provided by Actons Solicitors about Lasting Power of Attorney and by Alzheimer’s Society about their Dementia Friends programme. Read more about the event here.

Integrated Care System Partners AssemblyThe third ICS Partners Assembly took place on Monday 22 April at the John Fretwell Sporting Complex in Mansfield. The event was a success, bringing together 200 people from organisations across our health and care system, with opportunities to network, collaborate on ideas and hear inspirational stories and case studies. During the event we explored the Integrated Care Strategy. There were some great presentations and case studies from Bassetlaw Food Insecurity Network (BFIN), Integrated Neighbourhood Working (Community Care Transformation Programme), The Family Mentor Service in the City and Digital Notts. Place Partnership Director Fiona Callaghan also talked about proactive care. The slides from the event are available to download.

Dementia and carers marketplacePeople with dementia and their carers are invited to attend the Dementia and Carers Marketplace event which is being held on Thursday 16 May 2024 (10am – 2pm) at Rushcliffe Arena, Rugby Rd, West Bridgford, NG2 7HY The event is being hosted by Rushcliffe Dementia Action Network and will take place during Dementia Action Week 2024. Working Well East MidlandsWorking Well East Midlands is a free employment support service tailored to meet the individual’s needs. There is no pressure for people to take on a job they do not want to do. This is a personalised approach to helping people (over the age 18) who experience health problems, have a disability, have difficulties in sustaining work, or finding employment. The team work with people people who live in or are registered with a GP Practice in Nottingham City, Broxtowe, Gedling or Rushcliffe. New Young Carers serviceThis service provides free and confidential support for young carers in Nottinghamshire and Nottingham. Young Carers Notts is for young carers age 5-17 and offers group sessions, short breaks, social opportunities with other young carers, fun activities, and practical advice about caring for someone. Young Carers Notts also helps those working with young people to increase identification of young carers and promote the support available. Download the poster here.
