NW London COVID Medicine Delivery Unit (CMDU)


From 8 July 2024, the Covid-19 Medicines Delivery Unit (CMDU) Service changed from a Chelsea & Westminster hospital led service to a primary care led service.

We have two Providers that offer triage, assessment and treatment for COVID-19 positive patients to ensure access to COVID-19 treatments. These are as per below:

In addition, there are 16 community nominated pharmacies that will stock, dispense oral antiviral medication nirmatrelvir with ritonavir (Paxlovid®) with delivery an option (if required) to ensure local accessibility.

Eligibility Criteria:
  • Confirmed COVID-19 (LFT) within the last 5 days AND
  • Onset of symptomatic COVID-19 within the last 5* days AND
  • Patient has an increased risk for progression to severe COVID-19 as defined by supporting information on risk factors for progression to severe COVID‑19
  • or
  • aged 85 years and over or
  • have end-stage heart failure and have a long-term ventricular assistance device (a mechanical device that helps the heart pump blood) or
  • are on the organ transplant waiting list or
  • are a resident in a care home or already in hospital AND are:
o   aged 70 years and over or
o   who have a body mass index (BMI) of 35 kg/m2 or more or
o   diabetes or
o   heart failure 

Referral:

Patients should be referred to the service via email by their registered GP or 111 only. Patients need to be screened for eligibility (all providers are asked to use the screening tool first - and only ask the patient to contact the GP or 111 if the criterion is met or there is uncertainty) and referred using the form located in North West London Shared Folder – COVID19 Folder – NWL ICS COVID19 Paxlovid Screening Tool CMDU 2024 2025 (this is also the referral form) set up within the primary care IT systems. This tool is also attached for your information.

The service is available from Monday to Saturday.

If referred, please inform the patient that the service will contact them within the next 24 working hours to discuss whether they may benefit from treatment. If the patient does not hear from the service within 24 working hours (Mon-Sat), they should follow up.

Patients can also be referred into the service through 111.

Community Services, Mental Health Services and Secondary Care Services

If a patient in the community setting meets the eligible criterion for antiviral medication – please advise them to contact their registered GP or 111 to be referred to the service.

For secondary care patients currently monitored in a virtual ward and already under the care of a Respiratory Consultant – please contact the respiratory team  - as any evaluation and assessment and subsequent prescribing will be lead by this team.

Cataract services at the Western Eye Hospital


Two significant enhancements have been introduced to cataract services at the Western Eye Hospital. 

From the moment of referral, whether routine or straight forward, patients can expect a waiting time of one month until their surgery date. This efficient process ensures timely care and improved vision outcomes for our patients.

1. One-stop cataract service:
  • A new one-stop cataract service streamlines patient care, allowing for efficient pre-assessment, diagnosis, and surgery planning
  • Patients can now experience a seamless journey from referral to surgery, minimising waiting times and ensuring timely treatment
2. State-of-the-art high-volume cataract theatre:
  • The team are proud to unveil their cutting-edge cataract theatre equipped with advanced technology.
  • This purpose-built facility enables them to perform a higher volume of cataract procedures, ensuring quicker access to surgery for patients.

NEW Medical Certificate of Cause of Death (MCCD) books are arriving now


New MCCD books are being issued nationally from the Department of Health and Social Care. They started arriving in practices last week. They are arriving by FedEx from the Department of Health and Social Care (DHSC) and a signature will be required at the practice. You should receive your new MCCD book by the end of this week.

Any practices who have not received their MCCD books by mid-August should contact the DHSC directly by email: mccd@dhsc.gov.uk 

The new medical certificates of cause of death must be used from the date of statute 09 September 2024. Any old certificates used after this date will be rejected.

PLEASE DO NOT USE THE NEW MCCD BEFORE 9 SEPTEMBER

NW London diabetes enhanced services dashboard – June 2024

Polypharmacy translated 


ICHP are looking to roll out the translated polypharmacy – Are Your Medicines Working For You? across the wider NW London footprint.

The patient facing checklist aims to help patients and health professionals have conversations about medicines. Are Your Medicines Working For You? focuses on empowering patients to have discussions and structured medication reviews with their GP, practice pharmacist, or other health professional.

Keeping Well North West London is a psychological support service for all NHS and Healthcare staff who live or work in North West London. This includes the London Ambulance Service, volunteers and those providing care to others in the community.

Polypharmacy: medicines and the risk of falls in older people lunchtime masterclass

Falls are a frailty syndrome and a common presenting complaint for older patients attending healthcare facilities. As well as the human cost, falls are estimated to cost the NHS more than £2.3 billion per year.

Final dates released for polypharmacy action learning set workshops 

The National Polypharmacy Programme exclusively invites our NWL prescribers to join the Action Learning Set (ALS). The programme aims to build GP and prescribing health care professionals’ confidence in, and understanding of, the complex issue surrounding and stopping inappropriate medicines safely. Delegates need to attend all three sessions. The ALS with also help PCNs deliver the medicines optimisation elements of the new Directed Enhanced Services contract and contributes to the QoF.
